In this example we have a spreadsheet (created using Microsoft Excel) with a list of names , company names, fax numbers and e-mails. The Excel spreadsheet here is called the "data source"
We've created a Microsoft Word document (the template with form fields) which will be faxed to a list of potential clients.
We want the document (the template with the form fields) to contain personalized information such as the recipients name on the fax, but we don't want to type each one individually and manually print them so they can be faxed using a fax machine.
