This problem may occur due to two reasons- Hardware and software. Hardware reasons include bad jumper setting, hard drive controller failure, and hard drive internal physical problems like head crash or motor stop.

Software reasons include operating system corruption, sudden catastrophic failure and other hard disk driver failure. Any of these failures, no matter what is the type, can make the hard drive inaccessible and may cause data loss.
